What it means: A plan, deal, or arrangement failed to happen — usually after it seemed likely to succeed. Something that falls through was expected but didn't materialise.

What it implies: There's a sense of disappointment and often of something collapsing unexpectedly. Unlike "it didn't happen" (which is neutral), "it fell through" implies it was going to happen and then didn't.
